Endometriosis Surgery

Manage chronic pelvic pain and symptoms with targeted endometriosis surgery.

OUR PROCEDURE

Endometriosis is a medical condition where tissue similar to the lining of the uterus grows outside the uterine cavity. Endometriosis surgery is a procedure to remove or destroy these growths to relieve symptoms and manage the condition. The two main types of surgery are excision, which involves carefully cutting out the tissue, and ablation, which uses heat or other energy to destroy it.

This surgery is a crucial treatment option for individuals with moderate to severe endometriosis, especially when other treatments like medication have been ineffective. The goal of the surgery is to reduce chronic pain, bloating, and other debilitating symptoms, and in some cases, to improve fertility for those trying to conceive.

The Endometriosis Surgery is designed to:

  • Remove endometrial growths and adhesions.
  • Reduce chronic pelvic pain and cramping.
  • Improve fertility by clearing reproductive organs.
  • Minimize the growth of new endometrial tissue.
  • Restore normal organ function and anatomy.

What is Endometriosis?

Endometriosis is a condition in which tissues similar to the lining of the uterus (endometrium) grow out of place. It usually encompasses the ovaries and fallopian tubes and can affect the surrounding organs like the bladder and bowel. During the menstrual cycle, these cells respond to hormones, leading to pain and, over time, the formation of scars. It can also cause cysts in the ovaries called endometriomas.

Endometriosis Symptoms

  • Pelvic pain, especially at the period outset, beyond normal cramping
  • Pain with intercourse
  • Pain with bowel movement or urination
  • Infertility
  • Abnormal uterine bleeding

Some women have endometriosis without these symptoms.

Endometriosis diagnosis

The most effective, non-intrusive way to diagnose endometriosis is MRI Mapping (Endomapping).

Magnetic Resonance Imaging (MRI) is a non-invasive diagnostic tool that uses a powerful magnetic field and radio waves to generate detailed images of internal organs and tissues. While not always definitive for diagnosing endometriosis, MRI is particularly useful in detecting deep-infiltrating endometriosis and assessing the extent of the condition. It plays a crucial role in pre-surgical planning by providing surgeons with precise information about the size, location, and depth of endometrial implants, helping to improve surgical outcomes.

Endometriosis Treatment

Endometrial conditions can be effectively managed by ablation or endometriosis surgery. These therapies are designed to destroy or remove the lining of the uterus (endometrium). They are recommended for women who suffer from heavy or prolonged periods and are looking for a non-hormonal, minimally invasive solution. The procedure does not involve a hysterectomy and preserves the uterus, making it an excellent alternative for those who do not wish to undergo major surgery.

Excision vs. Ablation for Endometriosis

Endometrial excision surgery is a laparoscopic (keyhole) treatment to remove endometriosis lesions, whereas endometrial ablation uses heat to vaporize the abnormal tissues.

Types of Endometrial Ablation

There are several techniques used for endometrial ablation at Hospital Azar, including:

  • Radiofrequency Ablation: Uses electrical energy to heat and remove the uterine lining.
  • Cryoablation: Freezes and destroys endometrial tissue.
  • Thermal Balloon Ablation: A balloon filled with heated fluid is inserted into the uterus to burn the lining.
  • Microwave Ablation: Uses microwave energy to remove the endometrial layer.
  • Hydrothermal Ablation: Heated saline is used to destroy the lining.

How Are These Procedures Performed?
Steps to Endometriosis Surgery

Endometrial surgery is the surgical approach to curing endometriosis. It is performed by a highly skilled and specialized gynecologist surgeon using the gold-standard laparoscopic or minimally invasive approach. However, for cases involving extensive or deeply infiltrating disease, a laparotomy (traditional open surgery) may be required to ensure the thorough removal of all visible lesions.

  1. Preparation – Medical tests, fasting, and stopping certain medications before surgery.
  2. Anesthesia & Incision – You’ll be under general anesthesia; small (laparoscopy) or larger (open) incisions are made.
  3. Removal of Endometriosis – The surgeon removes or burns endometrial tissue and scar tissue.
  4. Closing & Recovery – Incisions are closed, and you wake up in recovery.
  5. Aftercare – Pain management, follow-ups, and lifestyle adjustments for healing.

For patients with severe bowel endometriosis, a temporary colostomy may be required after surgery to give the repaired portion of the bowel time to heal correctly.

Steps to Endometrial Ablation

Endometrial ablation is a quick, outpatient procedure typically taking 30 minutes or less. Here’s what to expect:

  1. Pre-Procedure Preparation: The doctor may prescribe medications to thin the endometrial lining. You will also need to avoid eating or drinking for a few hours before the procedure.
  2. Anesthesia Options: The procedure may be done under local, spinal, or general anesthesia, depending on your specific case.
  3. Ablation Process: A specialized device is inserted into the uterus through the cervix to remove or destroy the endometrial lining using the chosen technique.
  4. Recovery & Discharge: Since it is a minimally invasive treatment, most patients go home the same day with minimal discomfort.

Who Is a Candidate for Endometrial Therapy?

This procedure is ideal for women who:

  • Experience heavy or prolonged menstrual bleeding
  • Have no desire for future pregnancies
  • Do not have uterine cancer, infections, or fibroids that may interfere with the treatment
  • Are looking for a minimally invasive alternative to hysterectomy

Important Note: Endometrial ablation is not recommended for women who still wish to conceive, as pregnancy after the procedure can be risky.

Recovery & Aftercare

Most patients return to normal activities within 24 to 48 hours. Some common post-procedure effects include:

  • Light cramping and discomfort (similar to menstrual cramps)
  • Watery or bloody discharge for a few days to weeks
  • Changes in menstrual flow (lighter periods or no periods at all)

Your doctor will provide personalized aftercare instructions to ensure a smooth recovery. Follow-up appointments may be scheduled to monitor results.
